ABSTRACT

Introducción: el análisis detallado de las proporciones nasofaciales desempeña un papel fundamental para lograr la armonía facial e identificar desequilibrios; asi se dirige el tratamiento quirúrgico para lograr mejores resultados postoperatorios. El objetivo de este estudio fue determinar la prevalencia de pacientes sometidos a rinoplastia que requerían, además, un aumento del mentón. Materiales y métodos: se seleccionaron 100 pacientes voluntarios sometidos a rinoplastia entre los 17 y 55 años y se les realizó un estudio fotográfico preoperatorio. Se realizó un análisis facial del tercio inferior de la cara mediante tres métodos: González-Ulloa, Goode y Silver, y se hizo un análisis univariado y bivariado. Resultados: 100 pacientes voluntarios ingresaron al estudio; de estos, 7 fueron excluidos y quedaron 73 mujeres y 20 hombres; la edad mínima fue de 17 años y la máxima de 55 años, con un promedio de 28,4 años. La edad media de los hombres fue de 30,9 años y de las mujeres de 28,2 años. Del total de pacientes, 96,7 % de los pacientes cumplían con 2 o 3 métodos para aumento del mentón; de estos, 78,8 % eran mujeres y 21,1 % eran hombres. Conclusión: un análisis adecuado de las proporciones nasofaciales es fundamental para determinar los procedimientos necesarios para lograr un buen resultado quirúrgico y una mayor satisfacción del paciente. Estos métodos no sustituyen el juicio estético del cirujano; sin embargo, proporcionan un estándar objetivo para el diagnóstico de los desequilibrios faciales.


Introduction: Detailed analysis of nasofacial proportions plays a fundamental role in achieving facial harmony and identifying imbalances; thus, surgical treatment is directed to achieve better postoperative outcomes. The aim of this study was to determine the prevalence of patients undergoing rhinoplasty who also required chin augmentation. Materials and methods: 100 volunteer rhinoplasty patients between 17 and 55 years of age were selected and a preoperative photographic study was performed. Facial analysis of the lower third of the face was performed by three methods: Gonzalez-Ulloa, Goode and Silver, univariate and bivariate analysis was performed. Results: 100 voluntary patients entered the study, 7 of these were excluded, leaving 73 women and 20 men, the minimum age was 17 years and the maximum 55 years, with an average of 28.4 years. The mean age of the men was 30.9 years and of the women 28.2 years. Of the total number of patients, 96.7% of the patients com- plied with 2 or 3 methods for chin augmentation, of these 78.8% were women and 21.1% men. Conclusions: Adequate analysis of nasofacial proportions is essential to determine the procedures necessary to achieve a good surgical outcome and greater patient satisfaction. These methods do not replace the surgeon's aesthetic judgment; however, they provide an objective standard for the diagnosis of facial imbalances.

In clinical practice, we need to develop new tools to identify the residual cardiovascular risk after acute coronary syndrome (ACS). This study aimed to evaluate whether the monocyte to high-density lipoprotein cholesterol ratio (MHR) variation (ΔMHR) obtained during hospital admission (MHR1) and repeated in the first outpatient evaluation (MHR2) is a predictor of major adverse cardiovascular events (MACE) after ACS. One hundred ninety-one patients admitted for ACS were prospectively included. The ΔMHR was calculated by subtracting MHR1 from MHR2. Patients were followed for 166±38 days in which the occurrence of MACE was observed. The best cutoff for ΔMHR was zero (0), and individuals were divided into two groups: ΔMHR<0 (n=113) and ΔMHR≥0 (n=78). The presence of MACE was higher in the ΔMHR≥0 (22%) than in the ΔMHR<0 (7%), with a hazard ratio (HR) of 3.96 (95% confidence interval [CI]: 1.74-8.99; P=0.0004). After adjusting for confounders, ΔMHR≥0 remained an independent MACE predictor with an adjusted HR of 3.13 (95%CI: 1.35-7.26, P=0.008). In conclusion, our study showed that ΔMHR was an independent MACE predictor after ACS. Thus, ΔMHR is a potential marker of residual cardiovascular risk after ACS.

ABSTRACT

Restless legs syndrome (RLS) may severely affect the quality of life of patients. A deficient iron incorporation into the central nervous system has an important role in the pathophysiology of RLS. Severely affected patients may not respond to current therapeutic options. We report a preliminary experience with five patients with severe RLS and low serum ferritin levels who did not improve with oral iron. All were treated with 1 g of intravenous iron carboxymaltose. They experienced a marked improvement in symptoms, evident even during the first week of therapy that had persistent after up to two years of follow-up. A significant change in the RLS severity scale was observed after intravenous iron. Serum ferritin levels increased in all of them. Intravenous iron could be a therapeutic option for patients with severe RLS.

ABSTRACT

Emile Zola is one of the greatest writers in universal literature. In his important series of novels called "The Fortune of the Rougon-Macquart", Zola shows a surprising medical knowledge even though he did not have a formal medical education. We highlight not only his outstanding literary talent, but also the scientific relevance of the tremendous contribution to the medical field that can be extracted from his work. In this series, which describe the history of five generations within a large family suffering from neuropsychiatric and general pathologies, Zola emphasizes the hereditary component of several diseases. These observations probably place him as the first novelist who made an explicit emphasis on the power of inheritance in human behavior. He also mentions for the first time several medical aspects that were seldom addressed in the scientific literature of the time, demonstrating the genius of the writer, his outstanding power of observation and the rigorous preparation with which he wrote his work.

ABSTRACT

In veterinary medicine, the cell therapy is still unexplored and there are many unanswered questions that researchers tend to extrapolate to humans in an attempt to treat certain injuries. Investigating this subject in nonhuman primates turns out to be an unparalleled opportunity to better understand the dynamics of stem cells against some diseases. Thus, we aimed to compare the efficiency of bone marrow mononuclear cells (BMMCs) and mesenchymal stem cells (MSCs) from adipose tissue of Chlorocebus aethiops in induced bone injury. Ten animals were used, male adults subjected, to bone injury the iliac crests. The MSCs were isolated by and cultured. In an autologous manner, the BMMCs were infused in the right iliac crest, and MSCs from adipose tissue in the left iliac crest. After 4.8 months, the right iliac crests fully reconstructed, while left iliac crest continued to have obvious bone defects for up to 5.8 months after cell infusion. The best option for treatment of injuries with bone tissue loss in old world primates is to use autologous MSCs from adipose tissue, suggesting we can extrapolate the results to humans, since there is phylogenetic proximity between species.(AU)


Na medicina veterinária, a terapia celular ainda é inexplorada e há muitas perguntas não respondidas, o que leva os pesquisadores a uma tendência a estender a terapia para os seres humanos, na tentativa de tratar certas lesões. Investigar esse assunto em primatas não humanos revela-se uma oportunidade sem precedentes para compreender melhor a dinâmica das células-tronco contra algumas doenças. Assim, objetivou-se comparar a eficiência das células mononucleares de medula óssea (BMMCs) e das células-tronco mesenquimais (MSCs) do tecido adiposo de Chlorocebus aetiops na lesão óssea induzida. Foram utilizados 10 animais, adultos do sexo masculino, submetidos à lesão óssea nas cristas ilíacas. As MSCs foram isoladas e cultivadas; de forma autóloga, as BMMCs foram infundidas na crista ilíaca direita e as MSCs de tecido adiposo na crista ilíaca esquerda. Após 4,8 meses, a crista ilíaca direita foi totalmente reconstruída, enquanto a crista ilíaca esquerda continuou apresentando defeito ósseo evidente por até 5,8 meses após a infusão. A melhor opção para o tratamento de lesões com perda de tecido ósseo em primatas do Velho Mundo é a utilização de MSCs autólogas de tecido adiposo, sugerindo que se podem estender os resultados para seres humanos, uma vez que há proximidade filogenética entre as espécies.(AU)

ABSTRACT

Johann Sebastian Bach suffered during the last year of his life of a progressive visual defect despite two operations done by a famous but quite controversial English ocular surgeon of that time. The exact diagnosis of his ocular problems is unclear but cataracts and complicated glaucoma seem the most plausible. A septic complication following the ocular surgery could have weakened Bach's health leading to his death only three months after the last intervention. In this paper diverse less known aspects of Bach's disease and life are reported.

Abstract This is the first report on analysis of habitat complexity and heterogeneity of the Pantanal wetland. The Pantanal encompasses a peculiar mosaic of environments, being important to evaluate and monitor this area concerning conservation of biodiversity. Our objective was to indirectly measure the habitat complexity and heterogeneity of the mosaic forming the sub-regions of the Pantanal, by means of remote sensing. We obtained free images of Normalized Difference Vegetation Index (NDVI) from the sensor MODIS and calculated the mean value (complexity) and standard deviation (heterogeneity) for each sub-region in the years 2000, 2008 and 2015. The sub-regions of Poconé, Canoeira, Paraguai and Aquidauana presented the highest values of complexity (mean NDVI), between 0.69 and 0.64 in the evaluated years. The highest horizontal heterogeneity (NDVI standard deviation) was observed in the sub-region of Tuiuiú, with values of 0.19 in the years 2000 and 2015, and 0.21 in the year 2008. We concluded that the use of NDVI to estimate landscape parameters is an efficient tool for assessment and monitoring of the complexity and heterogeneity of the Pantanal habitats, applicable in other regions.


Resumo Este é o primeiro trabalho sobre análise da complexidade e heterogeneidade de habitats do Pantanal. O Pantanal é constituído por um mosaico de ambientes com características peculiares, sendo importante a avaliação e o monitoramento dessa área voltado para a conservação da biodiversidade. O objetivo do estudo é mensurar de forma indireta a complexidade e a heterogeneidade do mosaico de habitats os quais formam as sub-regiões do Pantanal, por meio do sensoriamento remoto. Foram obtidas, gratuitamente, imagens de índice de vegetação por diferença normalizada (NDVI) do sensor MODIS e calculado o valor de média (complexidade) e desvio padrão (heterogeneidade) para cada sub-região do Pantanal, para os anos de 2000, 2008 e 2015. Os pantanais de Poconé, Canoeira, Paraguai e Aquidauana são as regiões que apresentaram os maiores valores de complexidade (NDVI médio), variando entre 0.69 a 0.64 para os anos avaliados. Biological biomaterials for tissue engineering purposes can be produced through tissue and/or organ decellularization. The remaining extracellular matrix (ECM) must be acellular and preserve its proteins and physical features. Placentas are organs of great interest because they are discarded after birth and present large amounts of ECM. Protocols for decellularization are tissue-specific and have not been established for canine placentas yet. This study aimed at analyzing a favorable method for decellularization of maternal and fetal portions of canine placentas. Canine placentas were subjected to ten preliminary tests to analyze the efficacy of parameters such as the type of detergents, freezing temperatures and perfusion. Two protocols were chosen for further analyses using histology, scanning electron microscopy, immunofluorescence and DNA quantification. Sodium dodecyl sulfate (SDS) was the most effective detergent for cell removal. Freezing placentas before decellularization required longer periods of incubation in different detergents. Both perfusion and immersion methods were capable of removing cells. Placentas decellularized using Protocol I (1% SDS, 5 mM EDTA, 50 mM TRIS, and 0.5% antibiotic) preserved the ECM structure better, but Protocol I was less efficient to remove cells and DNA content from the ECM than Protocol II (1% SDS, 5 mM EDTA, 0.05% trypsin, and 0.5% antibiotic).

RESUMEN Objetivo: Determinar la percepción que tienen los pacientes hospitalizados en Servicio de Medicina Adulto de un hospital de alta complejidad, del cuidado humanizado que otorgan los profesionales de Enfermería y factores relacionados. Material y métodos: Estudio de corte transversal, correlacional, en una muestra de 171 pacientes hospitalizados en el servicio de medicina, durante el año 2014. Se aplicó una encuesta sociodemográfica y la escala Percepción de Comportamientos de Cuidado Humanizado de Enfermería (PCHE), segunda versión, adecuada transculturalmente a Chile. Resultados: 54,4% de los pacientes hospitalizados fueron de sexo femenino, media de edad de 56,2 años y 6,95 días de hospitalización, diagnóstico de ingreso de la mayoría fue enfermedad del sistema circulatorio (33,9%). El 86% de los pacientes hospitalizados considera que siempre recibe trato humanizado, 11,7% casi siempre, 1,8% algunas veces y un 0,6% nunca percibió un trato humanizado. Las cualidades en la entrega de cuidado humanizado por el profesional de enfermería que destacan los pacientes son las relacionadas a la categoría "Cualidades del hacer", es decir, perciben que son capaces de identificar sus necesidades, mantienen trato cordial, tienen buena disponibilidad cuando se les requiere y los educan. No existe asociación estadísticamente significativa entre el nivel de percepción de trato humanizado y las variables sociodemográficas. Conclusión: Los/as enfermeros/as de un hospital de alta complejidad respetan la dignidad humana al entregar cuidados humanizados, principalmente por la forma en que proporcionan los cuidados día a día. El estudio permite retroalimentación positiva al profesional que labora en la unidad, fortalecer su desem peño y la motivación para cuidar.


ABSTRACT Objective: To determine the perception of hospitalized patients in a high complexity hospital regarding the provision of humanized care by nursing professionals and related factors. Methods: Cross-sectional correlational study, a demographical survey and Perception Behavior Scale of Humanized Nursing Care (PCHE), second version cross-culturally adequate for Chile, was applied to a sample of 171 hospitalized patients during 2014. Results: 54.4% of hospitalized patients were female, mean age 56.2 years, and with a mean 6.95 days of hospitalization, admission diagnosis was mostly circulatory system disease (33.9%). 86% of hospitalized patients considered they always receive humane treatment, 11.7% almost always, 1.8% sometimes and 0.6% perceived never receiving humane treatment. The qualities in the delivery of humanized care by nursing professionals highlighted by patients are related to the category "Qualities of doing", in other words, patients perceive that the nurses are able to identify their needs, maintain cordiality, show availability when they are required, and are polite educate patients. No statistically significant association between perceived level of humane treatment and sociodemographic variables was found. Conclusion: The nurses in a high complexity hospital respect human dignity by delivering humanized care, mainly due to the way they provide day to day care. This study provides positive feedback to the professional working in the unit, strengthens their performance and motivation for caring.

ABSTRACT

The main adverse effects of dopaminergic drugs used in Parkinson's disease are hypotension, somnolence, hallucinations and impulse control disorder. Less common is leg edema. We report on a 68-year-old male receiving levodopa and pramipexole consulting for severe leg edema lasting two years, whose etiology was not ascertained with multiple lab tests. This edema subsided substantially when pramipexole was discontinued and the dose of levodopa was increased to treat motor symptoms.

ABSTRACT

Frontotemporal Dementia (FTD) and Motor Neuron Disease (MND) may share similar pathogenic mechanisms. An abnormal hexanucleotide expansion in C9orf72 gene is the most common genetic abnormality of these conditions and explains their concurrence in the same family. We report a 77-year-old female presenting with non-fluent aphasia leading to mutism and a mild Parkinsonism. A magnetic resonance imaging showed a severe atrophy of frontal and temporal lobes. Several family members of the patient suffered of atypical Parkinsonism, lateral amyotrophic sclerosis and dementia. We identified an abnormal hexanucleotide expansion in the C9orf72 gene in the proband. To the extent of our knowledge, this is the first time that this diagnosis is confirmed in our country. The knowledge of the genetic basis of neuro degenerative disorders improves diagnosis and opens expectatives for future treatments of these disabling conditions.

La música es un poderoso estímulo para nuestro cerebro y no solo una excelente fuente de disfrute, aprendizaje o relajante. Durante los últimos 20 años, hay nuevas evidencias considerables de la extensa Circuitos involucrados en el procesamiento de la música y en los procesos morfológicos, neuroquímicos y electrofisiológicos. Cambios que la música puede inducir. Nuestra población está envejeciendo y habrá un aumento en la prevalencia de enfermedades neurodegenerativas como las demencias que aún no tienen cura. Ahí está en consecuencia, una necesidad urgente de desarrollar tratamientos y actividades que puedan aliviar los síntomas. de la demencia. La evidencia acumulada muestra que las personas con demencia disfrutan de la música. La capacidad de responder a la música se conserva potencialmente incluso en las etapas tardías o graves de la demencia. Cuando la comunicación verbal puede haber cesado. A pesar de que los diversos estudios que reportan beneficios de la musicoterapia en condiciones como la demencia, el parkinson, la epilepsia, el cáncer, etc., tienen defectos metodológicos, la música puede tener un papel como herramienta terapéutica en estas condiciones. Aquí, analizamos la evidencia actual que puede apoyar el uso de la música en diferentes condiciones en medicina. (AU)


Music is a powerful stimulus to our brain and not only an excellent source of enjoyment, learning or relaxing. During the last 20 years there is new considerable evidence of the extensive neural circuits involved in music processing and the morphologic, neurochemical and electrophysiological changes that music can induce. Our population is aging and there will be an increase in the prevalence for neurodegenerative conditions like dementias that still have no cure. There is consequently an urgent need to develop treatments and activities that may alleviate the symptoms of dementia. Accumulating evidence shows that persons with dementia enjoy music, and their ability to respond to music is potentially preserved even in the late or severe stages of dementia when verbal communication may have ceased. Even though the diverse studies reporting benefits of music therapy in conditions such as Dementia, Parkinson, Epilepsy, Cancer, etc. have important methodologic flaws, Music can have a role as a therapeutic tool in these conditions. Herein, we analyze the current evidence that may support the use of Music in different conditions in Medicine. (AU)

ABSTRACT

Objetivo: Describir la prevalencia de istmocele como hallazgo incidental en pacientes con antecedente de cesárea y síntomas clínicos asociados. Métodos: Estudio descriptivo de corte transversal, mediante muestreo no probabilístico por conveniencia, en pacientes con antecedente de cesárea, programadas para histeroscopia entre noviembre de 2014 y marzo de 2015, en el servicio de cirugía endoscópica ginecológica del Hospital San José de Bogotá, Colombia. Resultados: 42 pacientes fueron elegibles para el estudio por su antecedente de cesárea y todas fueron incluidas. La frecuencia de istmocele fue 83,3% en la histeroscopia, con similar localización en istmo y cérvix. La principal característica clínica presentada por las pacientes fue hemorragia uterina anormal (85,7%), mientras que la menos frecuente fue infertilidad (7,1%). En las pacientes con presencia de istmocele se observó una mayor prevalencia de dismenorrea (65,7% vs. 42,9%), antecedente de 2 o más cesáreas (60% vs. 42,9%) y cesárea de urgencia (54,3% vs. 28,6%) comparadas con el grupo de pacientes sin istmocele, en este último grupo se advirtió que el 100% de las pacientes no tenían antecedente de trabajo de parto previo. En mujeres con antecedente de cesárea y presencia de síntomas como sangrado uterino anormal, dismenorrea, dolor pélvico, infertilidad y dispareunia, la frecuencia de istmocele diagnosticado por histeroscopia fue mayor del 80%. Conclusión: El istmocele se debe a la cicatrización anómala uterina posterior a una cesárea, se requieren otros estudios para determinar no solo la prevalencia sino los factores protectores que reduzcan su incidencia para tener un impacto positivo en este tipo de pacientes.


Objective: To describe the prevalence of isthmocele as an incidental finding in patients with a history of cesarean section and associated clinical symptoms. Methods: Descriptive cross-sectional study using nonprobabilistic sampling for convenience in patients with a history of cesarean section, scheduled for hysteroscopy between November 2014 and March 2015, in the gynecological endoscopic surgery service of the Hospital San José de Bogotá, Colombia. Results: 42 patients were eligible for the study because of their previous cesarean section and all were included. The frequency of isthmocele was 83.3% in hysteroscopy, with similar localization in the isthmus and cervix. The main clinical characteristic presented by the patients was abnormal uterine bleeding (85.7%), while the less common was infertility (7.1%). A higher prevalence of dysmenorrhea (65.7% vs. 42.9%) was observed in patients with isthmocele, a history of 2 or more cesareans (60% vs. 42.9%) and an emergency cesarean section (54, 3% vs. 28.6%) compared to the group of patients without isthmocele, in the latter group it was noted that 100% of the patients had no previous history of labor. In women with a history of cesarean section and presence of symptoms such as abnormal uterine bleeding, dysmenorrhea, pelvic pain, infertility and dyspareunia, the frequency of isthmocele diagnosed by hysteroscopy was greater than 80%. Conclusion: Isthmocele is due to abnormal uterine cicatrization after cesarean section, other studies are required to determine not only the prevalence but also the protective factors that reduce its incidence to have a positive impact on this kind of patients.

Background: Restless legs syndrome (RLS) affects 10% of the general population. Aim: To analyze a series of patients with a minimum follow-up period of four years, treated during an interval of 14 years. Material and Methods: Retrospective analysis of medical records of 200 patients assessed and followed by the authors at a private outpatient clinic. Results: Fifty patients aged 25 to 90 years (34 females), who had a mean follow-up of 6,3 years (range 4-14 years), were selected. Sixty percent responded to therapy that initially consisted in dopamine agonists in 78% of cases. Thirty four percent remained symptomatic and 4% worsened. RLS severity scale improved from an initial score of 19,2 to 12,5 at the last follow-up visit (p < 0.05). Thirty-three patients (66%) experienced an overall worsening of symptoms beyond pretreatment levels during follow-up. The strategies to overcome this augmentation were the change to another agonist, use of ligands such as pregabalin and gabapentin, opioids and iron. Low ferritin was common in most of the patients in whom it was measured (24 of 45 results), mainly in those with augmentation (p < 0,05). Six percent of patients treated with dopamine agonist developed an impulse control disorder. Conclusions: RLS is a treatable condition during a long period of follow-up in most patients. We found a high rate of potentiation at presentation which can be explained by the inadequate use of high doses of dopaminergic agents.

ABSTRACT

Among autoimmune encephalitides, a prevalent group are those associated with antibodies against the N-Methyl-D-aspartate receptor, which present with behavior abnormalities, psychosis, seizures and abnormal movements. A new variant, mediated by antibodies against the GABA-A receptor, was recen­tly described. We report a 66-years-old female with this form of encephalitis whose main manifestation was the presence of severe seizures leading to status epilepticus. The patient had a good response to immunomodulatory therapy with intravenous methylprednisolone, azathioprine and anticonvulsants. The laboratory tests initially detected anti-thyroid peroxidase antibodies which lead to the misdiagnosis of Hashimoto Encephalitis, which was ruled out after the detection of antibodies against GABA-A receptor. No malignancy was detected.

Introducción: La deficiencia de vitamina está siendo considerada como un problema de salud pública. Objetivos: Describir el estatus de la vitamina D en adolescentes del género femenino con obesidad y determinar factores de asociación. Diseño: Estudio observacional, descriptivo y analitico. Lugar: Lima Metropolitana. Participantes: Adolescentes mujeres. Intervenciones: Se estudió 111 adolescentes, 58 obesas y 53 no obesas, de acuerdo al indice de masa corporal. Se llevó a cabo una encuesta de alimentos dirigida a aquellos que proporcionaran vitamina D y se identificó los suplementos vitaminicos. Se les midió la circunferencia de la cintura y se tomó la presión arterial. Se les hizo dosaje en sangre venosa de glucosa, perfil lipidico y 25 hidroxivitamina D. El estatus de la vitamina D se determinó con los criterios de Holick. El análisis estadistico se efectuó con las pruebas de t-student, chi cuadrado y ANOVA, con un intervalo de confianza del 95 por ciento y un nivel de significancia < 0,05. Principales medidas de resultados: Promedios, intervalos de confianza y prevalencias. Resultados: Se encontró un 10,8 por ciento de adolescentes con deficiencia de vitamina D (< 20 ng/mL), 11,3 por ciento en las no obesas y 10,3 por ciento en las obesas. La ingesta promedio de vitamina D fue de 1,8 ug (IC 1,4 a 2,2) en las no obesas y 1,7 ug (IC 1,4 a 2,1) en las obesas. Mientras que en los deficientes la ingesta fue 2,3 ug (IC 1,9 a 2,9) y 1,7 ug (IC 1,4 a 2) en los no deficientes. No hubo asociación entre la vitamina D y los factores de riesgo cardiovascular. Conclusiones: Una de cada 10 adolescentes presentó deficiencia de vitamina D y dos de cada 10 insuficiencia.


Introduction: Vitamin deficiency is being considered a public health problem. Objectives: To describe vitamin D status in female adolescents with obesity. Design: Observational, descriptive, analytical study. Location: Metropolitan Lima. Participants: Teenagers. Interventions: According to body mass index, 111 adolescents were studied, 58 were obese and 53 non-obese. A food survey was performed including those containing vitamin D and vitamin supplements. Waist circumference and blood pressure were obtained. Glucose, lipid profile and 25 hydroxyvitamin D were determined in serum. Vitamin D status was determined by Holick criteria. Statistical analysis used student-t test, chi-square and ANOVA, with a 95 per cent confidence interval and a significance level of <0.05. Main outcome measures: Averages, confidence interval and prevalence. Results: Vitamin D deficiency (<20 ng/mL) was found in 10.8 per cent of adolescents, 11.3 per cent in non-obese and 10.3 per cent in the obese. Average vitamin D intake was 1.8 ug (95 per cent CI 1.4-2.2) in non-obese and 1.7 ug (95 per cent CI 1.4-2.1) in the obese. Intake was 2.3 ug (95 per cent CI 1.9-2.9) in those deficient and 1.7 ug (95 per cent CI 1.4-2) in the non-deficient. There was no association between vitamin D and cardiovascular risk factors. Conclusions: One out of each 10 adolescent women was vitamin D deficient and 2 of 10 had insufficient vitamin D.

RESUMO Os fitoterápicos à base de leite de janaguba (Himatanthus drasticus (Mart.) Plumel), usados para o tratamento de câncer, úlcera gástrica e outras doenças, são muito vendidos em mercados públicos de Fortaleza (CE). No entanto, registros mencionam que é comum a troca deste leite por látex de mangabeira (Hancornia speciosa Gomes). O trabalho objetivou avaliar a qualidade físico-química, química e microbiológica de amostras comerciais do leite de janaguba. Dez amostras comerciais foram adquiridas de um mercado de Fortaleza; quatro amostras autênticas de látex de janaguba foram obtidas da chapada do Araripe e uma amostra de látex de mangabeira foi obtida em Paracuru (CE). Foram determinados o aspecto geral, densidade, pH, resíduo seco, volume de sedimentação, perfil cromatográfico e qualidade microbiológica das amostras. Os resultados mostraram elevada contaminação microbiológica nas preparações comerciais e adulteração em seis destas amostras, o que aponta a urgente implantação de uma efetiva farmacovigilância dos fitoterápicos a fim de tornar seu consumo mais seguro e racional.


ABSTRACT The herbal medicines made from janaguba milk (Himatanthus drasticus (Mart.) Plumel), used to treat gastric ulcer, cancer and other diseases, and arewidely sold in the public markets of Fortaleza (CE). However, records mention that it is common to make fake copies of the milk by using mango tree latex (Hanconia speciosa Gomes). This study aimed to evaluate the chemical, physical-chemical and microbiological quality of commercial samples of janaguba milk. Ten commercial samples were purchased from a Fortaleza market; four authentic samples of janaguba latex were obtained from the Araripe plateau, and one mango tree latex sample was obtained in Paracuru (CE). We determined the general appearance, density, pH, dry residue, sedimentation volume, chromatographic profile and microbiological qualities. Results showed high microbiological contamination in the commercial preparations and evidence of tampering in six of these samples, which indicates the need for an urgent implementation of an effective pharmacovigilance strategy for herbal medicines in order to make its consumption more secure and rational.
